In many cases, forms need to be created in Slate. Below are our general policies as to when and why Slate forms are required. To learn how to create a form in Slate, check out our Slate form training. To view this training, you must log into Google through your illinois.edu email address.

Prospective Student Forms

If your form is for prospective students, it needs to be in Slate. This ensures that we are able to collect and store student information in one place.

Admitted Student Forms

If your form is for admitted students, it only needs to be in Slate if one of the following situations applies:

  • It is a registration form for an admitted event (to learn more about how to set up your event, see our event training)
  • You want to send communications to or pull a data report for people who have filled out the form
